All fees are current as of October 1st, 2008- All fees are subject to change at anytime. Payment is appreciated AT THE END OF EACH SESSION. In case of financial hardship, please consult Dr. Cheski for reduced payment arrangements. In this ministry of healing, we never turn sincere people away for financial reasons. We accept: Cash, Check, Traveler's Checks, and PayPal for all services provided. If you require other payment arrangements, please discuss it with the therapist at the time of your first appointment. We no longer accept credit cards, due to the lack of volume our processing company required - if you do plan to pay with a credit card, there is an ATM at the Walgreen's down the street on the corner of 6th Street and Oklahoma Avenue. Mental health insurance benefits vary widely from policy to policy, even within the same company. We cannot guarantee that your policy will cover your sessions here; but will be happy to call your insurance provider to request a pre-authorization for treatment and submit the claims if pre-authorization is granted. Naturopathic treatments and supplements are rarely covered by most insurance plans; however, some lab tests may be covered by your insurance policy. Ask Dr. Cheski for more details.
When you schedule an appointment, that time is reserved for you exclusively. We never double-book appointments; therefore, if for any reason, you are unable to keep a scheduled appointment, please call this office at least 24 hours prior to your appointment, if possible, to avoid being billed for the MISSED APPOINTMENT. The fee may be waived at the discretion of the doctor/therapist in cases of emergency or hazardous road conditions. To schedule an appointment with Dr. Terry Cheski call: 414-744-0300 or E-Mail her today! |
